Creator Alias: List the alias you would like to go by for all events. This will be used in place of your first and last name. It can be the same as your user name, if desired.

Studio/Group Name: List your studio or group affiliation. If listed, we will add this to the slide content for all contests.

Mailing Address: Enter your address here if you wish to specify postal mail for later correspondance. In select circumstances, we may elect to mail award compensation for attendees who cannot attend the convention.

Email Address: It is important that you provide a valid, working email address that you have access to it as email is the primary method we use to commuicate with you.

Proxy Recipient: List the first and last name of the person you would like to receive your award for you should you be unable to attend the awards ceremony. We will check a picture ID for all proxy recipients before allowing them to receive any award on your behalf.
